THE FACE OF

SOCIAL ANXIETY

It doesn't look much different on the outside, but to a social phobic - it sure feels like it!

Your heart pounds

        Your hands tremble

Your voice is low or absent

                        Your palms are sweaty

                                Your armpits are sweaty

                                    You feel alone & different

Thoughts of social phobics:

"I would sit near the phone for a while worrying about making a simple phone call."

"I never went to parties I was invited to. I would make up so many different excuses. I even made up an excuse that it was against my religion!"

"I felt like people would criticize my voice, my looks, my status, even my thoughts!"

"I had an irrational fear that people could see what I was thinking and people would criticize me about that behind my back."

"I felt relieved after finishing workdays but still thought about all the times during the day that I probably made a fool out of myself - even if there was no rational reason."

"I had to avoid people as much as possible."

WHAT IS SOCIAL PHOBIA?

Social phobia is an extreme fear of being judged by other people. It is sometimes confused with shyness but it is quite different. Social phobic people often fear people, public speaking, parties, meetings, classes and other situations that may include getting up in front of people or interacting with people. People with social phobia usually find it difficult to meet new people. They often hide or avoid crowds and social places.

A person with social phobia will most likely fear that people are going to make fun of them, criticize them, or negatively view them. They worry about looking like a "fool" in front of others. Sufferers often feel like people are staring at them.

Sometimes a social phobic person will not finish school, quit their job, not want to get married or close to anyone, and other situations that in avoiding seem to isolate and cause feelings of depression. Some studies show a higher rate of suicide among social phobic people.  Many people do not seek help with social phobia and resort to alcohol or substance abuse.

A social phobic most likely knows that he or she has an irrational fear but continues to feel the same way. They know deep down inside that people are not trying to humiliate them or critique them, but the fear is still overwhelming.

People suffering from social anxiety may resort to alcohol or drugs to rid themselves of the anxiety and make it through social situations. This can lead to severe substance abuse and alcoholism.

WHAT CAUSES SOCIAL PHOBIA?

Research on social phobia is still in progress. Some studies have found a possible genetic inheritance, yet environmental and learning or "social modeling" is still most likely a big cause of social phobia.

PUBLIC SPEAKING

Public speaking anxiety is the most common fear among social phobic individuals. If you suffer from public speaking fear, you may not be clinically diagnosed with social anxiety disorder, yet it still remains a pillar of social anxiety. Getting up in front of large audience members can be devastating for many people. It becomes a concerning issue though, when individuals fear talking in front of small groups of people (2 people and above). Remember: Shyness should not be confused with anxiety disorder. People who are shy might be timid to speak in front of 2 or more people, but they do not obsess about it and experience panic-like symptoms!

STAGE FRIGHT

Stage fright is a common fear among most individuals in the public speaking fear category. It is highly related to public speaking fears. Stage fright alone will not be categorized as a social anxiety disorder.

Performance anxiety is something many people have. Some super stars you would have never guessed had anxiety challenges. The king of rock and roll himself, (Elvis) had severe anxiety symptoms before every show. But his show still went on, and he is still the King!

The New Kids on the Block members, Jordan and Jonathan suffered from panic disorder. Learn more about their story here!

AGORAPHOBIA

Agoraphobia is a condition in which a person is so afraid of the anxiety feelings and the panic attacks that they avoid all situations in which there might be a chance of experiencing the anxiety attacks. This has lead many to conclude that agoraphobia is the fear of open spaces. It is not necessarily that, although the word was originally intended to mean that.

Agoraphobia usually develops over time. The individual may fear leaving the house because they do not want to come into contact with any situation or stimulus that can cause a panic attack.

SYMPTOMS OF AGORAPHOBIA

They symptoms of agoraphobia are panic attacks followed by the act of avoiding the situations that provoke the attack. Often the feared places are places in which it is quite hard to escape immediately. It is very common for an agoraphobic to fear the movie theater because it is not easy to get out. Yet, this is true for many other places from work to school and so on.

CAUSES OF AGORAPHOBIA

Studies have shown a link between agoraphobia and imbalances of serotonin. There may also be a genetic predisposition. For the most part, agoraphobia is learned though, through repeated avoidance of situations in which a panic attack may occur.

SHYNESS

A social phobic person is sometimes labeled as shy or introverted. Shy people may experience some anxiety from time to time in times that they feel a threat to their self-esteem, but the anxiety experienced by a "shy" person is not enough to make them avoid situations as much as a social phobic. A social phobic will go to great lengths to avoid situations that may be associated with the anxiety.

Shyness is thought to be partially genetic in nature. Some people are more reserved by default. Yet by learning the appropriate skills, you can overcome shyness and actually become the social butterfly within you - anxious to mingle!

EPIDEMIOLOGY OF SOCIAL PHOBIA

Social phobia seems to be as common as panic disorder. At least 5% of the world suffers from social anxiety and it is not specific to age, culture or socioeconomic status.

The statistics show that men and women develop social phobia just as frequent. The disorder may fluctuate over time and can benefit from cognitive-behavioral therapies as well as certain medications. The onset has been shown in some studies to occur most frequently in late teens. "Shy" children may also develop this disorder.

DIAGNOSES OF SOCIAL PHOBIA

Social phobia is usually diagnosed when the individual displays the following features:

A marked and persistent fear of being scrutinized or being evaluated negatively by other people in one or more social or performance situations. Situations commonly feared by social phobics are: speaking in public, eating in front of others, using public rest-rooms, being in social situations in which there is a chance of saying foolish things or things that may be criticized.

The fear involves acting in a way that will be embarrassing or humiliating and showing obvious symptoms of anxiety.

Exposure to the feared situation causes anxiety or panic attacks. Although extremely feared, it is recognized to be irrational.

The fear leads to marked distress during exposure to the social situation, or may lead to avoidance of that situation.

References: World Health Organization's (WHO) International Classification of Diseases (ICD)

Most of the population suffers from "normal" social anxiety and avoidance. Fears such as public speaking or stage fright do not necessarily justify the diagnosis of social phobia as long as the fear does not interfere with daily life.
